Alternative Plans

Transition Services

Transition Services refer to those programs and services provided by FCCPS specifically designed to prepare and support students with disabilities, Individual Education (IEP) and 504 Plans in their transition from high school to adult life.

Beginning at age 14, students receiving special education services must have a transition plan within their Individualized Education Plan.
