
Transition Services
Transition Services refer to those programs and services provided by FCCPS specifically designed to prepare and support students with disabilities, Individual Education (IEP) and 504 Plans in their transition from high school to adult life.
Beginning at age 14, students receiving special education services must have a transition plan within their Individualized Education Plan. With the support from parents, teachers and the Transition Coordinator, each plan in designed to best meet the needs and goals of the student to achieve postsecondary success. These goals may include working towards post-secondary education, training and/or independent living skills. Emphasis is placed on developing student self-determination and self-advocacy. All services are provided in a manner consistent with student-centered planning.

Providing instructional opportunities for students to acquire postsecondary skills (through the Education for Employment program)
Providing opportunities for career exploration and development of appropriate work behaviors (internships/supported employment)
Facilitating connections between adult services, vocational training, postsecondary providers and college and career counselors
